Pedal for Prizes Release

Through my participation in the activities of OLD BROOKLYN PEDAL FOR PRIZES, I hereby release the

organizers, sponsors, and volunteers of OLD BROOKLYN PEDAL FOR PRIZES, and any of their directors,

officers, agents, employees and volunteers from all claims of liability or every kind on account of such

participation. It is the specific intent and purpose of this instrument to release and discharge any and all

claims and causes of action of any kind or nature whatsoever, whether known or unknown and whether

specifically mentioned or not, which may exist or might be claimed to exist at or prior to the date of this

instrument and undersigned specifically waives any claim or right to assert that any cause of action or

alleged cause of action or claim or demand has been, through oversight or error or intentionally or

unintentionally, omitted from this release.

 

PHOTO RELEASE

Further, I hereby consent to the photographing of myself and the recording of my voice and the use of these photographs and/or recordings singularly or in conjunction with other photographs and/or recordings for advertising, publicity, commercial or other business purposes. I understand that the term “photograph” encompasses both still photographs, digital renderings and motion picture footage.

IN THE CASE OF A MINOR CHILD 

I voluntarily release any and all actions or causes of action for personal injury, property damage or other

loss arising as a result of participating, or receiving instructions in, the activity or activities of OLD

BROOKLYN PEDAL FOR PRIZES, wherever or however they may occur and for whatever period the

activities, or instructions may continue. The undersigned does for him/herself, minor children or youth

otherwise under my care or supervision, discharge and relinquish any action or causes of action, which

may hereafter arise for him/herself and for his/her estate, and agrees that under no circumstances will

he/she or his/her heirs, executors, administrators and assigns prosecute or in any way whatsoever,

present any claim for personal injury, property damage or wrongful death against OLD BROOKLYN

PEDAL FOR PRIZES, its organizers, sponsors or any of its officers, agents, volunteers or employees for any causes of action, whether arising by negligence or otherwise. I understand that minor children or youth under age 18 must wear a helmet. In signing, I confirm that minor children or youth are under my care or guardianship, and I am authorized to execute this release and permit the minor child or youths's participation in the activities of OLD BROOKLYN PEDAL FOR PRIZES.
